  • * Medical records shall be retained for at least 7 years after treatment has been completed
  • * Medical records of minors, shall be retained until the person becomes 19 years of age or until 7 years after treatment has been completed, whichever is longer.
  • * Any record undergoing federal or state audit shall be maintained until completion of the audit.
  • * Records relating to legal actions shall be maintained until completion of the legal action.
  • * Records relating to billing or collections shall be maintained for periods of time specified in s. DHS

Paper Copies:
$1 per page for the first 25 pages.
$0.75 per page for pages 26 to 50.
$0.50 per page for pages 51 to 100.
$0.30 per page for pages 101 and above.

Microfiche/Microfilm Copies: $1.50 per page.

X-ray Prints: $10 per image.
Certification of Copies: $8 (if the requester is not the patient or an authorized person).

Retrieval Fee: $20 (if the requester is not the patient or an authorized person).

Shipping Costs: Actual shipping costs plus any applicable taxes.

If a patient or his/her authorized representative requests medical records for use in appealing the denial of a Social Security disability insurance or Supplemental Security Income claim, the medical provider cannot charge “more than the amount that the federal Social Security Administration reimburses the department for copies of patient health care records.” Wisconsin Statutes § 146.83(1f)(am). 20 C.F.R § 401.95, showing a current SSA reimbursement rate of 10¢ per page, with the caveat that there is no charge if the total amount of copying does not exceed $25.
