Tennessee Medical Record Retention Law:

  • * Medical records must be kept for at least 10 years from the last time the physician or their supervisees had professional contact with the patient.
  • * Records for Minors: Keep these for at least 1 year after the minor reaches adulthood (18 years old) or for 10 years from the last contact, whichever is longer.
  • * Immunization Records: Keep these forever.
  • * Records for Incompetent Patients: Keep these forever.
  • * X-rays and Imaging: Keep for at least 4 years. If there are separate interpretive records, the images can be destroyed after 4 years. However, mammograms and their reports must be kept for 10 years.
  • * Disputed Records: Don’t destroy any records involved in ongoing disputes until the issue is resolved.

  • * For More Than 5 Pages: $20 for the first 5 pages, then $0.50 per additional page.
  • * Mailing Costs: You’re responsible for the actual cost of mailing the records.
  • * If a third-party company handles the copying, they must follow the same cost limits.
