1. Electronic Records:
First 30 Pages: $0.65 per page
Additional Pages: $0.50 per page
Clerical Fee: Up to $25 (for searching and handling)
Total Maximum Fee: $150 (includes clerical fee, but excludes postage and sales tax)
Printed Records:
First 30 Pages: $0.65 per page
Additional Pages: $0.50 per page
Clerical Fee: Up to $25 (for searching and handling)
Total Maximum Fee: $200 (includes clerical fee, but excludes postage and sales tax)
Note: Fees are adjusted annually based on inflation (Consumer Price Index).
2. No Charge for Referrals:
If your records are needed to continue treatment with another provider, you won’t be charged for the copies.
3. X-Ray Reproduction:
You can be charged only for the actual cost of reproducing X-rays, which includes materials, labor, and overhead costs.
