Arizona Medical Record Retention Law:

- * Keep patient records for at least six years after the last date the adult patient received medical or health care services from that provider.
- * Keep patient records for at least three years after the child's eighteenth birthday or for at least six years after the last date the child received medical or health care services from that provider, whichever date occurs later.

Health care providers or contractors may charge a reasonable fee for copying medical or payment records, unless the law states otherwise. Payment of these fees may be required upfront, except when the records are needed for ongoing care.
No fees will be charged for medical records when provided to:
Another health care provider for continuing patient care.
The patient for obtaining health care.
The patient's health care decision-maker for obtaining health care.
The Arizona Medical Board, Board of Osteopathic Examiners, or health officials when requesting records under section 36-662.
The patient or legal representative for appealing a Social Security benefit denial.
Additional requests in the same calendar year may incur fees, unless no records are found. A legal representative must submit form SSA-1696 to receive records free of charge.
