- * Records for adult patients must be maintained for a minimum of seven years from date of last patient encounter.
- * If a patient is a minor on the date of the last visit, then the physician must maintain the pediatric patient’s records for a minimum period of either seven years from the date of the last patient encounter or until the patient reaches the age of eighteen, whichever is the longer retention period.

Base Charge:
A base charge of no more than $15 per request for a hospital or clinic medical record.
Per Page Charges:
First 100 Pages: Up to $0.50 per page for each of the first 100 pages.
Pages Beyond 100: Up to $0.25 per page for each page in excess of 100 pages.
Exceptions to Fees:
No fee shall be charged when the record is requested to support a claim or appeal under:
The Social Security Act.
Any federal or state financial needs-based benefit program.
