As of April 30, 2026 the Ortholazer Dedham location has closed, and the Ortholazer Nashua location will be closing September 30, 2026. Your records will be transferred to Morgan Records Management, the Records Custodian. You can request copies of your records from this practice from Morgan Records Management using the form link below.
Digital copies of your records from Ortholazer can be requested by filling out the secure release form on this site — click the “Request My Records” button to get started. In accordance with applicable state law, a reasonable fee may be charged for the copying and delivery of records. You will receive an email confirmation once your request is received.

Morgan Records Management is the appointed records custodian for Ortholazer in Massachusetts. When the practice closes on September 30, 2026, its patient records will be transferred to us and you will be able to request copies at any time.
In accordance with applicable state law, you may be charged a reasonable fee for the copying and transmittal of records. The exact amount depends on the size of your file and your state's regulations.
Most states allow by law up to 30 days to process records requests. Due to the high volume of requests we receive, it can take up to 30 days. We do our best to process all requests sooner, as we know how important they are.
