Is there a fee for records?
If you are a patient of Dr. Weinblatt requesting a copy of your own records, there is no charge. Because the practice is closing and affirmatively terminating its relationships with patients, patients will not be charged for copies of their medical records.
If an insurance company, attorney, record-retrieval company, or another authorized third party makes its own request for records, that third party may be charged a fee permitted by applicable law.
How quickly will I receive my records?
We do have a standard 30 day turnaround timeframe, as required by applicable law. We do our best to process all records requests sooner as we know how important they are.
If information needed to process your request is missing, we will contact you promptly.
Can someone else request records on my behalf?
Yes, with proper authorization. Parents or guardians for minors, attorneys with a HIPAA-compliant release form, and others with notarized permission may submit requests on your behalf. The form will guide you through what is required.
Can I receive records electronically?
Yes. We can send records via secure email (HIPAA Compliant Electronic Transfer), or mail records on a USB Thumb Drive. You can choose which option you prefer on the form.
