Frequently asked questions

Is there a fee for records?

In accordance with applicable state law, you may be charged a reasonable fee for the copying and transmittal of records. The exact amount depends on the size of your file and your state's regulations.

How quickly will I receive my records?

Most states allow by law up to 30 days to process records requests. Due to the high volume of requests we receive, it can take up to 30 days. We do our best to process all requests sooner, as we know how important they are.

Can someone else request my records on my behalf?

Yes, with proper authorization. Parents or guardians for minors, attorneys with a HIPAA-compliant release form, and others with notarized permission may submit requests on your behalf. The form will guide you through what is required.

What if I don't have internet access?

Call our medical records team at (833) 888-0061 and we will mail you a paper request form.
