Digital copies of your records from Total Urgent Care LLC - Premier Health - TUC can be requested by filling out the secure release form on this site — click the “Request My Records” button to get started. In accordance with applicable state law, a reasonable fee may be charged for the copying and delivery of records. You will receive an email confirmation once your request is received.
