Morgan Records Management is the appointed records custodian for The Maryland Therapy Network in Maryland. Its patient records have been transferred to us, and you can request copies at any time using the request form on this page.
In accordance with applicable state law, you may be charged a reasonable fee for the copying and transmittal of records. The exact amount depends on the size of your file and your state's regulations.
Most states allow by law up to 30 days to process records requests. Due to the high volume of requests we receive, it can take up to 30 days. We do our best to process all requests sooner, as we know how important they are.
Yes, with proper authorization. Parents or guardians for minors, attorneys with a HIPAA-compliant release form, and others with notarized permission may submit requests on your behalf. The form will guide you through what is required.
