As of March 02, 2026, this practice is no longer providing patient care. Your medical records have been transferred to Morgan Records Management, the records custodian appointed by the practice. You can request copies of your records at any time.
Your records will be destroyed in accordance with laws regarding retention requirements – once the patient turns 18 or 6 years after the last date of services, whichever is longer.
