  • * It is recommended that Medicare/Medicaid medical records be kept for at least six (6) years after the physician received payment for the service.

  • The Ohio law allows health care providers or medical records companies to charge for copies of medical records, with specific fee limits. If the request is made by a party other than the patient or their authorized representative, the fees include an initial search fee of $16.84, $1.11 per page for the first ten pages, $0.57 per page for pages eleven through fifty, $0.23 per page for pages fifty-one and higher, and $1.87 per page for certain imaging records.

  • In Ohio, healthcare providers or medical records companies must provide one copy of a patient's medical record and subsequent treatment records at no charge to certain entities, including the Bureau of Workers' Compensation, the Industrial Commission, the Department of Medicaid, the Attorney General, and patients or their representatives if the records are needed to support a Social Security claim. Charges may apply for requests by other parties, with specific fee limits based on the type and amount of data being copied.
