- * Retention Period: Hospital records must be retained in their original form, microfilmed, or similarly reproduced for at least ten (10) years from the date the patient is discharged.
- * Graphic Matter (e.g., X-ray films, images):
Retention Period: Such materials, necessary for diagnostic or therapeutic purposes, must be retained for at least three (3) years from the date the patient is discharged.
Extended Retention: The retention period may be extended if a written request is made by the patient or someone legally acting on their behalf.

First 20 pages: $20.00
Next 80 pages: $1.00 per page
Pages beyond 100: $0.50 per page
Additional Charges:
Postage and Handling: An additional 10% of the total charge can be added.
Retrieving Archived Records: A $15.00 fee can be charged if the records need to be retrieved from an off-premises archive.
