Alabama Medical Record Retention Law:

Keep patient records for at least seven years after the last time the doctor (or other providers in the practice) saw the patient.
Exceptions:
- Immunization records not sent to the state registry: Keep these for at least two years after the minor turns 19 years old or for seven years after the last visit, whichever is longer.
- X-rays and other images: Keep them for at least five years. If there are separate reports interpreting these images, the images themselves can be destroyed after five years. However, keep mammograms and their reports for ten years.
- Medical records of minors: Keep these for at least two years after the minor turns 19 or for seven years after the last visit, whichever is longer.
- Records related to disputes: Don’t destroy any records if there’s an ongoing dispute about them. Keep them until the dispute is settled, as long as you were formally notified of the dispute before the retention period ends.

The cost for copying medical records can't be more than what state and federal laws allow. In Alabama, the law (Ala. Code § 12-21-6.1)
- $1 per page for the first 25 pages and up to 50 cents per page after that.
- If the records are mailed, you can also charge for the actual mailing costs.
- Can charge for the actual cost of copying X-rays and other special medical records.
